In this installment, players follow the journey of Enzo Favara, who escapes a life of indentured labor in the island’s sulfur mines. Through a twist of fate, he joins Don Torrisi’s powerful crime family, pledging loyalty under the family’s code of honor. The narrative explores the origins of organized crime, the challenges of survival, and the sacrifices demanded by loyalty.

The game builds on the franchise’s third-person action roots with grounded combat systems, including stealth mechanics, knife duels, and firearm battles. Visual design and character models aim to capture the atmosphere of the era, while cutscenes carry the story through cinematic sequences.

Two editions are available for purchase. The Standard Edition, priced at $49.99, includes the full game. The Digital Deluxe Edition, priced at $59.99, offers additional content via the Padrino Pack and Gatto Nero Pack. These packs provide extra weapons, outfits, vehicles, charms, and digital bonuses, including the official soundtrack and an artbook containing concept designs and developer notes.
